Ladder rework

I think the ladder item is too situational of an item to be useful in any realistic context.

I think either the maps need way more ladder points for it to be viable - like, way, way more, something like 10-20x more points than there are now in each map.

I’d honestly prefer if there was instead a throwable grapple that can detect edges and create a permanently placed rope for you to climb. Though I can see that being an issue to animate.
