Equipment going invisible

I’ve noticed that with repeated use of some of the equipment it can go invisible, whether that’s throwables or breaching charges. I think this happened when I used breaching charges and scrolled quickly to the smoke grenades after breaching. I don’t know if this could also just be the placeholder for empty throwables which you can still scroll to.
