Inconsistent Level progression

Occasionally I’ve noticed that the game kinda just stops registering your progression after a bit into the mission. I’ve collected all the intel on the map but the game has not acknowledged all of them but 5/13 ( for example ). A different instance was the main objective which was defusing the bomb. I had the check-mark that the bomb was defused however it was still a 0/1 Bombs defused and upon extraction, the mission was marked as failed.

That is most of it for now tho; This one can be quite game breaking; didn’t manage to record an instance tho.
